S5 Why risk control is a legal duty, not a good idea
- Identify hazards — find out what could cause harm.
- Assess risks — understand the nature of the harm, how serious it could be and how likely it is to happen.
- Control risks — implement the most effective control that is reasonably practicable.
- Review controls — make sure they are working, and fix them if they are not.
- Take reasonable care for your own health and safety.
- Take reasonable care that what you do (or do not do) does not adversely affect the health and safety of others.
- Comply, so far as you are reasonably able, with any reasonable instruction given by the PCBU.
- Co-operate with any reasonable policy or procedure of the PCBU that you have been notified of.

How the mapping is proved
Nobody types the matrix. Every block in the pack names the performance criteria, performance evidence and knowledge evidence it addresses, and the matrix is computed from those names at build time — 40 requirements, 19 taught, 0 assessed, 40 gaps. A requirement with nothing pointing at it cannot be hidden, because nobody wrote this table.
